Performers Release Form

I/We, do hereby give Dance for the Cure Charitable Foundation (DFTCCF) and those acting with the organization’s authority and permission, the unrestricted right and permission forever to copyright and use, re-use, publish, and republish photos and video footage of my/our interviews and performances from the 4th Annual Dance for the Cure Charity Ball held on October 13, 2013.

a) I understand that my image will be used in good taste and will not be used for pornographic or illegal purposes.

b) The work may be used with my name, or without my name. The work may be distorted, used in promotional material, advertisements, or in any other way the organization chooses to use the finished work, in print, on the web, or in any other public or private medium.

c) I will receive no compensation from profit that is made from the work, and I understand that all profit made from the work will benefit DFTCF.

d) I hereby relinquish any right that I may have to examine or approve the completed product or products or the advertising copy or printed matter that may be used in conjunction therewith or the use to which it may be applied.

e) I hereby affirm that I am over the age of majority and have the right to contract in my own name. I have read the above authorization, release and agreement, prior to its execution; I fully understand the contents thereof. This agreement shall be binding upon me and my heirs, legal representatives and assigns.
